So I have tested it again, and so far it seems to show the notice even if the no-index flag has been set prior to RankMath being installed/enabled. I will keep an eye on this issue and see if there is a particular way to reproduce this issue, and will follow up if it comes up again.
Regarding the error messages showing in RankMath interface, since you weren’t able to consistently access my local environment, I just tried it in a standard environment, and I’m unable to reproduce those errors. So, I’m guessing this might be something specific to my local environment.
So, essentially, the issue(s) seems to be resolved now. One thing I would strongly recommend though is making that notice WAY more prominent. For a VAST majority of site owners/web masters, having the no-index flag represents EXTREME risk, and the current subtle notice is not representing that sufficiently. Check how Yoast does that for example.

We just installed Yoast SEO on a local installation to verify the message they are showing and it seems that the wording is different and the color in the message is red.
We are going to share this feedback with our developers and if they decide to modify the notification to be more prominent we’ll let you know of the same.
Regarding the particular issue that started this ticket, if you notice a particular use case where the notification is not showing you can always ping us again in this ticket.
