Can you please share that CSV file with us so we can debug the issue fully and advise if any changes are needed for the import to work? You can upload the CSV file you are trying to import via this tool and share the URL here.
Meanwhile, the system that imports the redirections rely on scheduled actions to import the CSV data into the website. Please install the WP Crontrol plugin and navigate to WP Dashboard > Tools > Cron Events to make sure that the Cron Jobs are working fine on your site.
