Please note that we only report the 404s and unfortunately, we do not have information on how or why they are generated. You can get further insight into them by enabling the advanced monitor: https://rankmath.com/kb/monitor-404-errors/#advanced-404-monitor
That will give you more information like referrer and user-agent. You can use that information to track down the source or find out if it is just bots generating those errors.
You can fix these errors by redirecting them to a related page on your website:
https://rankmath.com/kb/fix-404-errors/
Or you can exclude the URLs from WordPress Dashboard > Rank Math > General Settings > 404 Monitor: https://rankmath.com/kb/general-settings/#exclude-paths
Regarding your homepage URL in the 404 monitor, when you see this, it means that some bots are trying to access this URL: https://yourdomain.com/yourdomain.com
This is because all of the 404 errors are relative to the homepage, so you can safely ignore those and go about your day.
If the error was on your homepage, you would see only a forward slash in the log /
.

but I’m asking about the duplicated 404 URLs, should I ignore them ever time I see them or what?
for example today I fixed the URL of mysite.com/example99 >> to mysite.com/example
so why I see tomorrow or even after awhile the same error come again >> mysite.com/example99

If you have already redirected the URL, then it shouldn’t return a 404 error and shouldn’t be logged again in the 404 Monitor. However, the old log for that URL will not clear in the monitor, but you will still see the URL there. However, you won’t see new hits for that.
