For initial troubleshooting, please check if you are using a Firewall as it may prevent saving the changes.
If you are using a Firewall, please whitelist this route:
/wp-json/rankmath/v1/updateMeta
You can check this link for more information:

You may need to have this route whitelisted for all user roles on the website for the users without admin access to be able to save the metadata on the website.
If you are using a security plugin on the website, make sure that the route is whitelisted on the plugin for all roles.

Editors are able to save the settings, so presumably this means the firewall settings are fine ??
I have checked the console and it is reporting 403 (Forbidden)

This means that our routes are being blocked by one of your security services or Cloudflare. You can follow this guide to whitelist Rank Math in Cloudflare or Wordfence or Sucuri. The process is similar in other services too.
