We have reviewed the majority of your posts and all of the failed tests were all correct.

For example, on the above post we could not find the focus keyword in any of your subheadings, i.e. H2/H3/H4. We could also not any image with your focus keyword as an alt tag, neither could we find any internal link on the page.
