There’s limitation of 60 characters for the title that Google allows to show in search result page. The current title is more than 60 characters. That’s one of the major reason why it’s not showing %separator% %site%. I would suggest try reducing the characters or test it out in other page with title less than 60 characters.
Once, you’ve a page with title and site, try testing it out in URL inspection in Google Search console and see if it shows the proper titles and description.

Okay, I have shorted the sitename to “Precision PT” so it’s less characters.
The thing is, some of my pages were under 60 characters, for example: https://imgur.com/PylrTkD
“Thomas Parkhill – Precision Physical Therapy & Sports Rehab” is 59 characters and the separator and sitename still didn’t display.

Please note that Google sometimes ignores the custom meta info altogether and shows something from the page’s content that matches the search intent better. In this case, the best you can do is optimize your meta title and description to try and match the intent of the search/keyword.
